Updated Informed Consent Documents
The following informed consent documents have been updated:
- HRP-502: TEMPLATE - Informed Consent - The template has been updated to include a link to the Informed Consent Guidance and Standard Language resource.
- Informed Consent Guidance and Standard Language (formerly Informed Consent Guidance)- This guidance document now includes the Standard Language (formerly Language Text Resource) resource.
- Parental Permission form - The parental permission form has been updated to include a link to the Informed Consent Guidance and Standard Language resource and includes the addition of the assent signature block.
