The IRB has discontinued the use of the electronic form (eForm) for all amendment submissions. In its place investigators should use HRP-213: FORM - Amendment to request any modifications to an IRB approved study.Ìý HRP-213 is available on the Forms & Templates page of the IRB website and downloadable from within the Amendment submission in .
