Form Change for Continuing Review Submission
Tthe IRB has discontinued the use of the electronic form (eForm) for all continuing review submissions. In its place investigators should use HRP-212: FORM - Continuing/Final Review Progress Report to report on the status of the study. HRP-212 is available on the Forms & Templates page of the IRB website and downloadable from within the CR submission in . If you have any questions please contact the IRB Office.
