New Informed Consent Template
The IRB Office published an updated version of the informed consent template. The template and instructions for uploading this form in eRA have been posted to the Forms & Templates page.
For your convenience, these forms are also available in the system as well. When you have added the Consent Form as an attachment, you may click on theÌý icon to access an instructional Consent Sheet which describes how to download, edit and upload the new consent form from within .
